Read Chapter 2 & the Hypertension Encounter Form . Search the Internet for more information on how to ensure data quality, such as can be found at American Health Information Management Association (AHIMA) link: www.ahima.org/ -1. Write 1 introduction paragraph (4 sentences). 2. Create 1 table that examines the disease state of hypertension. The table should include 30 different data fields pertaining to the patient’s demographics, financials, and diagnostics. Fields might include general information such as weight and height, and specific information to the particular disease. Use the Sample Form (file uploaded) as a basis for creating your table, and answer the table. The Sample Form has only 22 data fields, add 8 fields or more, and you can also change the fields. Keep in mind that not all data fields should be treated the same and different data types can be entered for each field, such as text, numerical, etc. 3. List and explain 3 specific alert mechanisms and 3 triggering mechanisms. These may be used in the event that data of a time sensitive or clinically sensitive nature would be entered, such as a very high heart rate or fraudulent insurance identification number. 4. List and describe the 10 characteristics of data quality of AHIMA (file uploaded) in your own words. 5. In 1 paragraph (4 sentences or more), describe what the consequences could be in the event of non-compliance for the data fields of hypertension. 6. Write 1 concluding paragraph (4 sentences).
